Waktu bekerja telah tamat. Zack masih tidak berganjak dari tempat duduknya. Dia gigih mencari maklumat tentang penyakit Rheumatoid Arthritis di internet.
What Is Rheumatoid Arthritis?
Arthritis means inflammation in a joint. Joint inflammation causes redness, warmth, swelling, and pain within the joint.
Rheumatoid arthritis is a type of chronic arthritis that occurs in joints on both sides of the body (such as both hands, both wrists, or both knees). This symmetry helps distinguish rheumatoid arthritis from other types of arthritis.
What Are the Symptoms of Rheumatoid Arthritis?
Symptoms of rheumatoid arthritis are:
• Joint pain and swelling
• Stiffness, especially in the morning or after sitting for long periods
• Fatigue
Rheumatoid arthritis affects everyone differently. For some, joint symptoms develop gradually over several years. In others, rheumatoid arthritis may develop quickly, while other people may have rheumatoid arthritis for a short time and then have a time with no symptoms, called remission.
What Causes Rheumatoid Arthritis?
The cause of rheumatoid arthritis is unknown. It is thought to be due to a combination of genetic, environmental, and hormonal factors. With rheumatoid arthritis, something seems to trigger the immune system to attack the joints and sometimes other organs. Some theories suggest that a virus or bacteria may alter the immune system, causing it to attack the joints. Other theories suggest that smoking may lead to rheumatoid arthritis.
Research hasn't found exactly what role genetics plays in rheumatoid arthritis. Some people do seem to have a genetic or inherited factor that increases their chance of developing rheumatoid arthritis.

Rheumatoid Arthritis Progression
Each person's rheumatoid arthritis is unique, and the disease affects each person differently. Over the long-term, though, there are a few common patterns.
•Long remissions. Remission means near-disappearance of symptoms without an actual cure. About 5% to 10% of people diagnosed with rheumatoid arthritis have a sudden onset of the illness, but then have no symptoms for many years, even decades.
• Intermittent symptoms. About 15% of people with rheumatoid arthritis have disease that waxes and wanes slowly. They have periods of low or no symptoms that can last months between flare-ups.
• Progressive rheumatoid arthritis. Unfortunately, that leaves the majority of people, who have the most common and serious form of
rheumatoid arthritis. Because it's progressive, it requires a long-term treatment plan and a coordinated medical team to manage the treatment and slow or stop progression.
How Is Rheumatoid Arthritis Treated?
There are many ways to treat rheumatoid arthritis. Treatments include medications, rest and exercise, and surgery to correct damage to the joint.
The type of treatment will depend on several things, including the person's age, overall health, medical history, and severity of the arthritis.
Rheumatoid Arthritis Medications
There are many rheumatoid arthritis medications available to lower joint pain, swelling, and inflammation. Some of these drugs prevent or slow the progression of the disease.
Drugs that offer relief of arthritis symptoms (joint pain, stiffness, and swelling) include:
• Anti-inflammatory painkillers, such as aspirin, ibuprofen, or naproxen
• Topical (applied directly to the skin) pain relievers
• Corticosteroids, such as prednisone
• Narcotic pain relievers
There are also many strong medications called disease-modifying antirheumatic drugs (DMARDs), which work by interfering with or suppressing the immune system's attack on the joints. They include:
• Plaquenil (originally used to treat malaria)
• Immune suppression drugs, such as methotrexate, Imuran, and Cytoxan
• Biologic treatments, such as Enbrel, Humira, Remicade, Orencia, and Rituxan
• Other drugs, such as Azulfidine, Arava, and Xeljanz
Can Rheumatoid Arthritis Be Cured?
• Although there isn't a cure for rheumatoid arthritis, early, aggressive treatment has been shown to help prevent disability.
Don't overlook the affect your rheumatoid arthritis has on your mental health. If you are having trouble coping, seek help. Psychologists, social workers, and psychiatrists can help you deal with the struggle of living with the long-term uncertainty and limitations of rheumatoid arthritis.

“Hai Zack. Tak balik lagi?”
Teguran kali ini menyebabkan Zack mengangkat muka. Senyuman manis Zareen muncul depan mata. Hilang sebentar semua maklumat yang dibaca tadi.
“Hai Zareen." Zack telan air liur. "Err…maksud saya…Cik Reen.” Zack tergagap-gagap. Suka-suka hati panggil boss dengan nama saja.
Zareen kekal tersenyum.
“Banyak sangat ke kerja yang saya bagi sampai kena stayback?” Soal Zareen lagi, separuh bergurau.
“Kalau pasal Cik Reen, stayback sampai esok pun saya sanggup.” Zack terlajak kata sekali lagi. Jawapan ala-ala nak mengayat awek begitu tak sepatutnya keluar dari mulutnya ketika ini.
ZACK, JAGA SIKIT MULUT TU! Otak Zack memberi arahan dengan pantas.
“Err..ehem. Kerja memang banyak. Tapi tak adalah sampai kena stayback pun.” Zack mengubah ayat.
Zareen tersenyum sambil menggelengkan kepala.
“Cik Reen nak balik dah ke?”
Zareen mengangguk.
“Balik dengan siapa?”
YANG KAU SIBUK KENAPA? SUKA HATI DIALAH NAK BALIK DENGAN SIAPA. Otak Zack menegur lagi.
“Balik sorang ajelah. Kenapa? Nak hantar saya balik?” Balas Zareen.
NAK! NAK! NAK!
“Kalau saya hantar nanti, marah pulak boyfriend Cik Reen nanti…”
Zareen tergelak. Zack memandang dengan senyum sumbing lantas menarik nafas. Kalaulah Zareen jadi bini, hari-hari dia nak suruh Zareen gelak macam ini.
“Saya balik dululah.” Kata Zareen, tersipu sedikit setelah menyedari yang Zack merenungnya agak lama.
“Okay. Baik-baik jalan Cik Reen. Jatuh bangun sendiri.” Zack membuat lawak zaman Pak Pandir.
“Iyalah Zack. Assalamualaikum.”
“Waalaikummusalam.”
Zack memerhatikan Zareen sehingga hilang dari pandangan dan kemudian, menarik nafas panjang.
Zack kembali menumpukan perhatian kepada skrin komputer.

“Hi Kate.” Tegur Zack.
Kate tersenyum memandang Zack. Rokok di jarinya sudah dihisap separuh.
“Smoking can make her condition worse.” Kata-kata Audrey bergema dalam otak Zack. Zack mengurut tengkuknya. Betapa dia ingin sekali menegur Kate supaya berhenti merokok. Tapi, Zack tahu, itu tak akan mendatangkan hasil. Apatah lagi, dia sendiri turut sama merokok.
Zack menyalakan sebatang rokok untuk dirinya sendiri lalu duduk di sebelah Kate.
“How’s your day?” Tanya Zack, memulakan perbualan.
“Do you need to ask?” Kate menjawab dengan soalan. Zack tersengih.
Masing-masing menghembuskan asap rokok.
“Eh…I nak tanyalah. Berapa lama you kenal Audrey?” Tanya Zack.
“Lama.”
“I tak tahu pun yang Audrey tu…doktor. Dia tak nampak pun macam doktor.” Kata Zack.
“Then…you rasa, Audrey kerja apa?” Kate menyoal balas.
“Supermodel.”
Kate ketawa besar menyebabkan Zack turut ketawa sama.
“Oh God! This is funny. Audrey will kill you if she heard this.” Kata Kate dengan tawa yang masih bersisa.
“Audrey ada beritahu I pasal penyakit you.” Setelah menghembus asap rokok buat kali ke empat, Zack bersuara.
Wajah Kate serta-merta bertukar mendung.
“What else she told you?” Soal Kate.
“A lot.” Jawab Zack. Kate menggelengkan kepala.
“Idiot.” Perlahan Kate menuturkan perkataan itu. Wajahnya kekal muram.
“You should not be mad. She's care about you.” Kata Zack.
“Ya..I know. She’s like a sister to me. Overprotective one, I must say.” Balas Kate.
"Kate.."
"Yup..."
“You should stop smoking though. Smoking can make your condition worse.” Akhirnya, Zack cakap jua perkara yang ada dalam kepalanya.
“Oh...shut up! I heard it from Audrey already. Stop smoking. You’ll get worse. Yada…yada...yada…”
Zack tergelak sedikit.
“Kalau you dah tahu, kenapa tak nak berhenti?” Soal Zack.
“Kenapa you tak nak berhenti?” Kate mengulangi soalan Zack. Kening Zack terangkat.
“We are not talking about me. Lagipun, I tak sakit.”
“Stupid excuse. Smoking is bad for everybody. Does not matter if you are sakit or not. ” Balas Kate.

“Okay. Why don't we stop smoking. Together.” Zack membuat cadangan spontan.
Kate hampir ternganga. “Are you serious?”
“Yes.” Zack angguk. Kate terkebil-kebil.
“What makes you suddenly want to quit?” Soal Kate.
“Because…errr…hhhm. Smoking is bad for health.” Jawab Zack. Kate menggelengkan kepala. Kurang percaya dengan alasan Zack. “
“Idiot. Do you expect me to believe that?”
Zack tersengih.
“Don’t tell me you want to quit because you....want me to quit. That is…bullshit too.” Kate menyambung kata-katanya.
“Well…” Zack menarik nafas.
“There is this girl. I kind of like her…” Zack berkata perlahan, secara tak langsung, membuat pengakuan.
“Uuuuuuuuuuuuuuuu……” Kate tersenyum sinis. “And….you want to quite because…”
“Dia tak suka orang hisap rokok.” Jawab Zack.
“Oh….” Kate terangguk-angguk.
“So, you want to quit because she tak suka orang hisap rokok?”
Zack senyum sumbing, mengiyakan kata-kata Kate.
“Who is she? Do I know her?” Tanya Kate, cuba mengorek rahsia.
“Maybe…” Zack cuba bermain teka-teki. Kening Kate terangkat.
“Wait…is it…” Kate kelihatan teruja namun ayatnya tidak dihabiskan. Zack menanti Kate menghabiskan ayat.
“I think I know who you are talking about but…never mind.” Kata Kate.
“So, you hope that if you quit smoking, she will like you too?” Soal Kate.
“I hope so but I don’t think it will happen. Dia dah ada boyfriend. So…” Zack menghembus nafas kasar.
“Oh…” Kate terangguk-angguk lagi.
“Then, kenapa you still nak stop smoking?”
Zack angkat bahu.
“I know it’s wrong to wish something bad to happen but I…” Zack menarik nafas. “ I hope that she will break-up with her boyfriend… soon...or tomorrow… or now…”
Kate senyum sinis sambil menggelengkan kepala.
“And then, I boleh masuk line…” Sambung Zack.
“Line? What line?”
Zack memutarkan biji mata.
“I mean, if she broke-up…then I can…”
“Okay..okay. Got it. I know what you mean…” Kata Kate, tersenyum.

“Maybe. Maybe not. Anyway, that’s not the point. Smoking is bad for health so let’s stop.” Balas Zack.
Kate menghembus nafas kasar. “I did try..few times. Hundred times… but failed.”
“Me too. But maybe, if we can do it together..who knows…”
Kate mendiamkan diri, seperti memikirkan sesuatu.
“Come on, Kate. Apa yang you nak fikir lagi?”
“Okay, let’s do it.” Kate berkata, penuh semangat kali ini. “But…what if I feel the urge to smoke?”
Zack terdiam. Turut sama memikirkan perkara itu. Untuk berhenti, bukan mudah. Sukar untuk menahan jika rasa gian untuk merokok datang.
Zack tersenyum. “Wait here…” Zack bangun dari duduknya.
“Hey, where are you going?” Soal Kate terpinga-pinga. Tapi Zack tak menjawab.
Lima minit kemudian, Zack kembali dengan sepuluh batang gula-gula Chupa Chups pelbagai perisa di tangan.
“Five for you…” Zack menghulurkan lima batang Chupa Chups pada Kate.
“What the hell is this for?” Kate kehairanan tapi tangan dihulur juga menyambut gula-gula itu.
“Kalau you terasa nak smoking, makan ni.” Jawab Zack. Kate ketawa.
“Are you sure, this is gonna work?”
Zack angkat bahu sambil tersenyum.
“This is weird. Awesomely weird…”
Zack ketawa.
“I want the strawberry…” Pinta Kate apabila melihat perisa gula-gulanya.
Zack memutarkan matanya. Perisa strawberi dihulurkan kepada Kate. Kate menggantikannya dengan perisa anggur.
“Bagi I kotak rokok you.” Ujar Zack.
“What for?”
“Bagi ajelah.”
Kate menurut. Kotak rokok dihulurkan kepada Zack. Zack turut mengeluarkan kotak rokoknya lalu dibuang ke dalam tong sampah berdekatan.
“Damn it. I baru belilah!” Kate terjerit.

Kate memandang gula-gula di tangan dan muka Zack bergilir-gilir dan kemudian menggelengkan kepala.
“Okay. So now, let’s promise to each other. Stop smoking.” Kata Kate.
“Stop smoking.” Balas Zack. Mereka melagakan sebatang gula-gula Chupa Chups seperti melagakan gelas.
Tiba-tiba Kate ketawa dengan kuat.
“What?” Soal Zack, pelik.
“You know what, I got a name. Let’s call this promise… a Chup Chup Promise.” Jawab Kate lalu ketawa lagi.
“What? Chup Chup? That’s sounds cheesy though…”
“Who cares? Siapa suruh namakan gula-gula ni Chupa Chups?” Kate masih lagi ketawa. Zack turut ketawa.
“Whateverlah makcik. Asalkan kau bahagia…” Kata Zack.
“Chup Chup Promise. My goodness. I feel like five years old kid…” Kate menyambung kata dan tawa.
